District Nurses
The district nurses visit housebound patients in their own homes and assess their healthcare needs, providing and co-ordinating a holistic and quality-driven service to patients within the practice. They work closely with carers, GP’s, hospitals, Palliative Services, Social Work, Physiotherapy, Occupational Therapy and Podiatry Services.
Health Visitors
The Health Visitor Team at the surgery work alongside the doctors and other members of the Primary Health Care team. Their primary aim is to promote general health and well being in individuals and families. They visit patients at home, but also provide services within the practice which covers a wide range of social and healthcare problems. These services include: health advice to under 5’s, immunisations, developmental assessments and support services for young mothers.
Pharmacy
The Pharmacy team work alongside our MDT to provide a comprehensive review of medication. They are often vital in advising our staff on particular medication changes and can be a useful point of contact for patients looking to discuss any medications issues.
Wellbeing Nurse
Our wellbeing nurse is an experienced psychiatric nurse based in practices across Perth who will provide help and support to those experiencing mental health issues. She has a wealth of experience, including a vast knowledge of third party organisations within the local area to utilise for support. Being a psychiatric nurse, she is very experienced at dealing with mental health concerns and is open to all patients at the practice who feel they could benefit from talking to someone about their mental health issues.
Physiotherapy
First Contact Physiotherapy is a community service within Tayside that provides quick access to a Musculoskeletal assessment for patients in our local area. This involves review of your particular joint pain/strain by the physiotherapy team, some form of intervention (usually exercises) to get your treatment started, and further onward referral or investigation as they see fit. This was introduced to speed up the process of caring for patients with acute joint aches and pains and is extremely beneficial to patients.
Social Prescriber
We have a social prescriber in the practice that your GP/clinician may feel you would benefit from seeing. They are experienced professionals with a wealth of knowledge about services within the community that can help patients to re-engage with the community. This can range anywhere from help with loneliness/isolation, through to exercise, and to help with finances. Your GP/clinician will discuss this with you if they feel this is a relevant service for you.
